Initial Comments:
Based on the findings of an off site follow-up survey completed on July 28, 2022, Community Care Inc. was found to have corrected the deficiency and to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiency was cited as a result of an onsite unannounced State Re-licensure survey conducted on March 22, 2022.
